Denise Berlin, DO
Dr. Berlin brings roots grown deep in the local community and is passionate about whole-body health. She graduated as valedictorian at St. Joseph’s College in Rensselaer, IN, with a Bachelor of Science in Biology-Chemistry in 1995. In 1999, Denise completed medical school at the University of Osteopathic Medicine and Health Sciences in Des Moines, Iowa. After that, she completed both her osteopathic internship and family practice residency at Meadville Medical Center in 2002.
For twenty-two years, Denise primarily practiced Family Medicine in a rural health clinic at Canadohta Lake Health Center, where she also taught LECOM students on practical rotations. She has maintained her physician license from 2000 until the present, even while taking necessary time off to raise her growing family.
Denise has a wide range of passions, including preventive medicine, advanced lipid testing for heart health, and nutritional counseling. Primarily, though, Denise sees her patients as whole persons and works carefully with them to pursue total health.
Joe Graybill, MSN, FNP-C
Joe Graybill is a board-certified Family Nurse Practitioner with over 15 years of experience in healthcare, including the last five as a CRNP. His expertise spans diagnosing and treating both children and adults.
Joe began his nursing journey at the University of Pittsburgh at Titusville, where he graduated magna cum laude with an Associate’s Degree in Nursing in 2010. His began working critical care after graduation, earning Critical Care Registered Nurse (CCRN) certification in 2012. His career has included roles in volunteer EMS, Emergency and Critical Care, and Trauma Program Management.
In 2016, Joe graduated summa cum laude with a Bachelor’s Degree in Nursing from Clarion University, knowing the goal was to attend graduate school. He went on to earn his Master’s Degree in Nursing from Edinboro University in 2019. Since then, Joe has served as a board-certified Family Nurse Practitioner in both inpatient and outpatient settings.
A lifelong resident of Little Cooley, PA, Joe and his wife Leah, along with their seven children are now the second generation maintaining the family farm. He continues to work the land, as a hobby raising grass-fed beef and hair sheep utilizing regenerative practices.
Suzie Mattocks, RN
Suzannah Mattocks is a Registered Nurse and Board-Certified Lactation Consultant. She has worked within the hospital setting in various departments, including Inpatient Mental Health, Med-Surg, Maternity, and Pediatrics. She has worked in Maternity from 2011 to the present. She is passionate about supporting and educating parents about the birthing process and offers personalized and sensitive care to breastfeeding mothers and their babies.
Leah Graybill, Office Manager
Leah Graybill, Office Manager, is delighted to join her husband, Joe, to see a years-long vision come to life. While raising seven children, farming using a sustainable method of agriculture, and pouring into our community and neighbors, she has found the time and energy to manage a family practice that she believes can serve the community by filling the niche of simple, patient-directed care with a touch of education of the lifestyle choices that can lead to wholeness.
Leah is busy behind the scenes, keeping the practice functioning smoothly. It is her goal to provide seamless care options and follow-up without the hassle and confusion that healthcare has become known for.
Tammy Stutzman, Receptionist
As receptionist, Tammy brings order to the chaos. A long-time native of Crawford County, she has a passion for healthy communities, healthy families, and walking with others in their pursuit of health. With many years of medical study, as well as many years of front desk experience, Tammy is a valuable resource, filling many roles at Homestead.
